Current members of the Los Angeles County Disaster Communications Service are required to complete an additional form to maintain their membership. Those that joined after July 2011 completed this form as part of their processing and do not have to complete it again. Print the form Complete the form by entering the Date, signing your Signature, and printing your Full Name Scan the signed form and email the scanned form to K-01, or Mail or deliver the original of the completed form to the address below.
